The ‘WEST Living Well Programme’ will operate for six month period from October 2020, using the £100,000 to provide direct practical and timely support to over 400 vulnerable persons in the form of nutritious meals and social engagement resource packs, that will be delivered to the vulnerable persons own home. To effectively implement this programme Lakeland Community Care Limited are working collaboratively with a range of other organisations including, the Western Health & Social Care Trust Primary Care; Older Peoples Services Team, Fermanagh Rural Community Network, South West Age Partnership and OAK Healthy Living Centre. This will ensure that as far as possible the most vulnerable persons living within the communities of Fermanagh & Omagh District are identified, receive essential support through this programme and that other required support interventions can be put in place.
